Centra Community Health Needs Assessment

To provide a deeper understanding of our communities’ health needs and how we can address them, every three years Centra conducts Community Health Needs Assessments (CHNA). In 2021, we are partnering with Centra on these efforts and we would like to hear from you! To thank you for filling out the survey, you can enter a drawing for a chance to receive a $25 gift certificate to Walmart.
